RN Case Manager

Job Title: RN Case Manager

Location:Santa Rosa, California, 95405

Contract duration: 13 weeks

Shift:5X8 -hours

Schedule: Days

Key Responsibilities:

• Assess patients for case management needs and eligibility for services.
• Develop, implement, and evaluate individualized treatment and discharge plans.
• Coordinate interdisciplinary team meetings and family meetings to support patient care goals.
• Collaborate with physicians, nurses, social workers, and other care providers.
• Ensure compliance with CMS, NCQA, and The Joint Commission standards.
• Coordinate with insurance providers, HMOs, and IPAs for approvals and authorizations.
• Arrange post-acute services including DME, SNF, home health, and transportation.
• Educate patients and families regarding care plans, resources, and follow-up care.
• Advocate for patient needs while ensuring cost-effective care management.
• Accurately document all assessments, interventions, and outcomes in Epic.
• Monitor patient progress and update plans of care as appropriate.

Requirements:

• Minimum 2 years of nursing experience
• Previous Charge Experience: Required
• Travel Experience: Required
• Charting System: Epic (required)
• Community Hospital Experience: Preferred
• LTAC Experience: Preferred
• Trauma Level I or II Experience: Preferred
• Certifications: BLS (required)
• Work History: Must have current work history within the last 2 years
