This is a unique position providing a secondary level of care and case management for Sisters living in and around the East SF Bay Area.

About this role:
As part of the Province Health and Well-Being department, this position provides nursing care management to the Sisters of the Holy Names of Jesus and Mary (SNJM) who are living primarily in the East Bay area of California. The RN consults and advises with each Sister regarding individual healthcare needs and services and performs all essential functions of care coordination. This position proactively contributes to the enhancement of the Sister’s understanding of her own health and well-being and respectfully assists the Sister in meeting her healthcare needs.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Advises individual Sisters with healthcare needs, including supporting Sisters in their independence and with their individual preferences.
  2. Performs essential functions of care coordination which may include assessment, development of health and related needs-based plan, implementation, monitoring, evaluation, and adjustment. 
  3. Transports Sisters to medical appointments when needed and provides advocacy in the current healthcare environment, including accompaniment requested by a Sister for important appointments, during health transitions, for medical testing, surgery, etc.
  4. Transports/arranges transport/or meets Sister at the Emergency Department. Assists in care coordination and discharge planning.
  5. Serves as a resource to Sister and her chosen support person(s), especially during times of health transitions, serious medical conditions, and surgery.
  6. Provides and/or assists with education of Sisters about health and wellness topics.
  7. Actively participates in periodic Health and Well-Being meetings, community health care meetings, and workshops/seminars related to care coordination. Develops connections with organizations and educational offerings to foster aging well.
  8. Provides occasional direct nursing care services as needed, with the approval of the Director of Healthcare Services.
  9. Assists Sisters in communicating with healthcare providers.
  10. Maintains positive, professional working relationships with healthcare providers, physicians, discharge planners, home health agencies, staff at facilities, hospice, etc. Attends facility care conferences as appropriate.
  11. Documents pertinent clinical information and updates in the SNJM database in a timely manner.
  12. In collaboration with the Director of Healthcare Services/Manager of Healthcare Services, facilitates, coordinates, and monitors use of home care and other community- based services.
  13. Works closely with Manager of Healthcare Services and California Benefits Coordinator in areas relating to insurance, benefits, driving assessments, and special equipment.

Education:                  Graduation from an accredited nursing program; BSN preferred.
Experience:                 RN with 5 – 8 years’ experience, including home health care management experience, or equivalent. Geriatric experience preferred.
Certification:              Current RN license-State of California

  • Thorough knowledge and understanding of the concepts of aging and concerns of the elderly population.
  • Demonstrated ability to communicate professionally, empathetically, and compassionately with elder population who may be ill, disabled, hospitalized, and/or experiencing loss.
  • Demonstrated knowledge of and experience coordinating community resources in the East Bay area.
  • Ability to develop and maintain collaborative relationships with SNJM Sisters, staff, and external contacts.
  • High degree of flexibility and adaptability to shifting priorities and time
  • Excellent organizational skills and ability to prioritize and execute tasks in a timely
  • Demonstrated written and verbal skills to ensure effective
  • Ability to respond to urgent and/or stressful situations appropriately and
  • Proficient knowledge of Microsoft Office and other computer software programs including, but not limited to Word, Excel, Outlook.
  • Valid driver’s license and personal vehicle; good driving
  • Ability to travel frequently within East Bay and Campbell areas to fulfill job

Position: Full-Time, 40 hrs/week

Pay Range: $46-$52/hr
